Requests and Service Survey

There are 2 ways we monitor the WHIC traffic. 1) The Tell Us What You Think form, and 2) the Pamphlet Request form. Forms are located on the countertop located on your immediate left as you enter the center. Leave the completed form in the folder in the divider on Julie’s Desk.

The purpose of monitoring WHIC traffic is to be able to note trends in information requests, which will help us improve ways we can better serve our clients.

Whenever a client enters the center and uses any of the resources, even just to sit and read a magazine, please ask them to fill out the Tell Us What You Think form.

If a pamphlet is requested, have the client fill out a Pamphlet Request form.

If a client or employee calls for anything, please fill out the Tell Us What You Think form with their contact information and question. You can leave this form on Julie's chair so she knows to get back to them.
